Description:
This session will include two facilities sharing unique ways they’ve implemented processes to help improve patient care. The first presentation will focus on how standardization and implementation of protocols in multidisciplinary care settings are important to improve care coordination, safety, and patient outcomes. Come see how a mid-size academic medical center was able to utilize registry data to successfully design a multidisciplinary quality committee, implement new protocols, and quantify success for the lab.
The second presentation will explore best practices and strategies for standardizing, assessing, and addressing risk related to Cath/PCI procedures. It will focus on integrating best practices into documentation tools and then utilizing those tools to guide an individualized plan of care through all phases of care - pre, intra and post procedure. Optimizing documentation tools to capture Cath/PCI risk model elements will also be shared as a strategy to accurately reflect patient acuity and improve efficiency of data abstraction.
