Video Transcription
To code yes for bleeding gastrointestinal, there must be documentation the patient experienced gastrointestinal bleeding that was observed and documented in the medical record. Then, once identified, there must be any one of the following, a hemoglobin drop of three or more, a transfusion, or a procedural intervention surgery at the bleeding site to reverse or stop or correct the bleeding. While there may be documentation that criteria one or two or three took place, when there is no documentation reflecting the patient experienced gastrointestinal bleeding, this bleeding event would not be captured.
Video Summary
In order to code for gastrointestinal bleeding, there must be clear documentation of the bleeding in the medical record, alongside specific criteria such as a drop in hemoglobin levels, a transfusion, or a surgical intervention at the bleeding site. If there is no documented evidence of gastrointestinal bleeding, the event will not be coded as such. It is crucial for healthcare providers to accurately document and report instances of gastrointestinal bleeding to ensure proper coding and treatment.
