When the patient has been diagnosed with heart failure, the registry wants to know the patient's  New York Heart Association functional classification based on physician documentation at the time of the current procedure.  The NYHA functional classification must be specifically documented in the medical record as it cannot be left for interpretation by the abstractor based upon patient symptoms.  Classifications range from the least limiting, or class 1, to the most limiting, or class 4.
